{"created":"2023-05-15T11:23:38.785267+00:00","id":3387,"links":{},"metadata":{"_buckets":{"deposit":"d7239749-440c-4f3b-8bda-bda86b756b60"},"_deposit":{"created_by":1,"id":"3387","owners":[1],"pid":{"revision_id":0,"type":"depid","value":"3387"},"status":"published"},"_oai":{"id":"oai:showa.repo.nii.ac.jp:00003387","sets":["1:6:273:274"]},"author_link":["11478","11475","11477","11474","11476","11479","11473"],"item_10002_biblio_info_7":{"attribute_name":"書誌情報","attribute_value_mlt":[{"bibliographicIssueDates":{"bibliographicIssueDate":"2020-06"},"bibliographicIssueNumber":"1","bibliographicPageEnd":"34","bibliographicPageStart":"29","bibliographicVolumeNumber":"80","bibliographic_titles":[{"bibliographic_title":"昭和学士会雑誌"}]}]},"item_10002_description_6":{"attribute_name":"抄録","attribute_value_mlt":[{"subitem_description":"水銀は第12族元素に属している原子番号80の元素で，常温・常圧下で固体にならない唯一の金属である．感作能を有し，水銀体温計破損後に全身性接触皮膚炎と称される特有の皮疹を生じることが知られている．水銀の感作率および感作の原因を検討する目的で，29年間のパッチテスト結果を検討した．1990年3月より2018年6月までに昭和大学病院附属東病院，横浜市北部病院，藤が丘病院の皮膚科外来を受診し，パッチテストを施行された1,683名（男355名，女1,328名，平均年齢44.7，SD±18.8歳）を対象とした．0.05％塩化第二水銀水溶液試薬をパッチテスター「トリイ」ないしはアレジーズパッチテストチャンバークリア（SmartPractice）を用いて背部健常皮膚に貼布し，2日後に除去した．判定はICDRG（International Contact Dermatitis Research Group）基準に基づいて貼布2，3日後（1990年3月〜2011年3月），貼布2，3，7日後（2011年4月〜）に施行し，前者は貼布3日後，後者では貼布7日後に＋〜＋＋＋と判定された者を陽性とした．陽性反応が認められたのは130名で陽性率は7.7％であった．陽性者の平均年齢は37.8歳で，そのうち20歳未満が10％，30歳未満で41％を占めており，61％が40歳未満であった．施行時期別では1990年代には13.3％であった陽性率が，5.4％（2000年代），4.8％（2010年〜）と低下したが，これは40歳以下における陽性率の著明な低下による変化であった．以上の結果より水銀感作は低年齢で成立していると考えられた．低年齢で接触しうる水銀含有物としてワクチン内の防腐剤であるチメロサール，水銀体温計，歯科金属のアマルガム，消毒薬のマーキュロクロム，朱肉を考えた．チメロサールについては塩化第二水銀と同時にパッチテストを施行した136名中，両者に陽性反応を示した例は認められなかった．水銀体温計は広汎に用いられていたが，破損しなければ水銀に暴露されない点から感作原になった例は多くないと推察した．この両者とアマルガムに関しては同様に用いられていたはずの欧米諸国における水銀感作率が低い点からも感作原として否定的であった．したがって水銀感作はマーキュロクロムや朱肉により経皮的に成立した可能性が高いと考えた．水銀の感作率は減少傾向にあるが，パッチテストの陽性率はジャパニーズスタンダードアレルゲン2015に含まれている他のアレルゲンと比較すると決して低いとは言えず，本邦では引き続き重要なアレルゲンとして扱う必要があると考えられる．","subitem_description_type":"Abstract"}]},"item_10002_publisher_8":{"attribute_name":"出版者","attribute_value_mlt":[{"subitem_publisher":"昭和大学学士会"}]},"item_10002_relation_14":{"attribute_name":"DOI","attribute_value_mlt":[{"subitem_relation_type_id":{"subitem_relation_type_id_text":"10.14930/jshowaunivsoc.80.29"}}]},"item_10002_source_id_9":{"attribute_name":"ISSN","attribute_value_mlt":[{"subitem_source_identifier":"2188-529X"}]},"item_10002_version_type_20":{"attribute_name":"著者版フラグ","attribute_value_mlt":[{"subitem_version_type":"VoR"}]},"item_creator":{"attribute_name":"著者","attribute_type":"creator","attribute_value_mlt":[{"creatorNames":[{"creatorName":"平野, 由似"}],"nameIdentifiers":[{}]},{"creatorNames":[{"creatorName":"笠, ゆりな"}],"nameIdentifiers":[{}]},{"creatorNames":[{"creatorName":"宇野, 裕和"}],"nameIdentifiers":[{}]},{"creatorNames":[{"creatorName":"大歳, 晋平"}],"nameIdentifiers":[{}]},{"creatorNames":[{"creatorName":"保坂, 浩臣"}],"nameIdentifiers":[{}]},{"creatorNames":[{"creatorName":"末木, 博彦"}],"nameIdentifiers":[{}]},{"creatorNames":[{"creatorName":"中田, 土起丈"}],"nameIdentifiers":[{}]}]},"item_files":{"attribute_name":"ファイル情報","attribute_type":"file","attribute_value_mlt":[{"accessrole":"open_date","date":[{"dateType":"Available","dateValue":"2020-06-25"}],"displaytype":"detail","filename":"80_29.pdf","filesize":[{"value":"369.5 kB"}],"format":"application/pdf","licensetype":"license_note","mimetype":"application/pdf","url":{"label":"80_29.pdf","url":"https://showa.repo.nii.ac.jp/record/3387/files/80_29.pdf"},"version_id":"bbfca14c-161b-4e66-9d57-b905c2c56b96"}]},"item_language":{"attribute_name":"言語","attribute_value_mlt":[{"subitem_language":"jpn"}]},"item_resource_type":{"attribute_name":"資源タイプ","attribute_value_mlt":[{"resourcetype":"departmental bulletin paper"}]},"item_title":"水銀感作率の推移に関する検討","item_titles":{"attribute_name":"タイトル","attribute_value_mlt":[{"subitem_title":"水銀感作率の推移に関する検討"}]},"item_type_id":"10002","owner":"1","path":["274"],"pubdate":{"attribute_name":"公開日","attribute_value":"2020-06-25"},"publish_date":"2020-06-25","publish_status":"0","recid":"3387","relation_version_is_last":true,"title":["水銀感作率の推移に関する検討"],"weko_creator_id":"1","weko_shared_id":-1},"updated":"2023-05-15T12:31:54.646999+00:00"}
