{"created":"2023-05-15T11:21:28.435479+00:00","id":140,"links":{},"metadata":{"_buckets":{"deposit":"70f71988-50c3-4f4a-9aab-f801721d1b21"},"_deposit":{"created_by":1,"id":"140","owners":[1],"pid":{"revision_id":0,"type":"depid","value":"140"},"status":"published"},"_oai":{"id":"oai:showa.repo.nii.ac.jp:00000140","sets":["1:6:12:13"]},"author_link":["669","670","667","668","672","671"],"item_10002_biblio_info_7":{"attribute_name":"書誌情報","attribute_value_mlt":[{"bibliographicIssueDates":{"bibliographicIssueDate":"2017-02"},"bibliographicIssueNumber":"1","bibliographicPageEnd":"81","bibliographicPageStart":"74","bibliographicVolumeNumber":"77","bibliographic_titles":[{"bibliographic_title":"昭和学士会雑誌"}]}]},"item_10002_description_6":{"attribute_name":"抄録","attribute_value_mlt":[{"subitem_description":"近年の周産期医療の進歩により，極低出生体重児の救命率が向上し，児の長期予後に関心が集まっている．なかでも学童期に達した児の呼吸機能には潜在的な障害があると指摘されている．従来，小児の呼吸機能評価はスパイロメトリーが行われてきたが，手技が難しいなどの課題があった．一方で，強制オシレーション法（Forced Oscillation Technique: FOT）は，安静呼吸で測定でき直接的に呼吸抵抗や肺コンプライアンスが測定できる新規の呼吸機能検査法である．今回われわれは極低出生体重児の学童期の呼吸機能を，FOTを用いて初めて評価し検討した．2013年5月から2016年3月までに極低出生体重児のフォローアップ目的で昭和大学病院小児科を受診した学童21人を対象とし，2種類の呼吸機能検査（FOTとスパイロメトリー）を実施した．FOTにはMostgraph-01Ⓡを用いた．また，患者背景因子を診療録と保護者へのアンケート調査から得て，FOT測定値との関連性を検討した．統計解析は，Wilcoxon t検定を用い，両側検定でp＜0.05を統計学的有意と判断した．解析対象は21人，男児12人女児9人で，年齢は中央値8.5歳（6.4～13.2歳），身長は123cm（107.5～157.0cm），出生体重は927g （483～1,458g）だった．不当軽量児（SGA）は15人（71.4％），慢性肺疾患（CLD）は7人（33.3％），修正36週に酸素が投与されていたCLDは4人（19.0％），妊娠高血圧症候群（PIH）は5人（23.8％）だった．FOTは全員が正しい手順に則って行うことができたが，スパイロメトリーが出来たのは13人（61.9％）であった．FOT測定値は全ての項目で中央値が標準値を超え，特に％X5，％Fresは20％以上高値だった．また，FOT測定値のいずれかが標準値よりも50％以上高値を示した児は11人おり，そのうち10人で％X5が高値だった．このX5高値群（％X5≧150）は，対照群に比べCLDの割合が多い傾向があった．スパイロメトリー測定値は，％PEFRおよび％V25が標準値より80％以下で，拘束性障害が2人，閉塞性障害が1人，混合性障害は認めなかった．患者背景因子別にFOT測定値を検討したところ，PIHの有無において％R5，％R20に有意差を認めた．極低出生体重児の学童期の呼吸機能は呼吸抵抗も肺コンプライアンスも，既往歴のない学童よりも悪い傾向が認められた．特にX5高値群でCLDが多かったことは，X5がCLDによる肺コンプライアンス低下を鋭敏に検知している可能性が考えられた．また，スパイロメトリーで呼吸機能を評価できなかった症例も，FOTでは評価できたことから，今後，小児呼吸機能検査としてFOTがますます普及することが望まれる．","subitem_description_type":"Abstract"}]},"item_10002_publisher_8":{"attribute_name":"出版者","attribute_value_mlt":[{"subitem_publisher":"昭和大学学士会"}]},"item_10002_relation_14":{"attribute_name":"DOI","attribute_value_mlt":[{"subitem_relation_type_id":{"subitem_relation_type_id_text":"10.14930/jshowaunivsoc.77.74"}}]},"item_10002_source_id_9":{"attribute_name":"ISSN","attribute_value_mlt":[{"subitem_source_identifier":"2187-719X"}]},"item_10002_version_type_20":{"attribute_name":"著者版フラグ","attribute_value_mlt":[{"subitem_version_type":"VoR"}]},"item_creator":{"attribute_name":"著者","attribute_type":"creator","attribute_value_mlt":[{"creatorNames":[{"creatorName":"山崎, 明香"}],"nameIdentifiers":[{}]},{"creatorNames":[{"creatorName":"北條, 彰"}],"nameIdentifiers":[{}]},{"creatorNames":[{"creatorName":"宮沢, 篤生"}],"nameIdentifiers":[{}]},{"creatorNames":[{"creatorName":"神谷, 太郎"}],"nameIdentifiers":[{}]},{"creatorNames":[{"creatorName":"今井, 孝成"}],"nameIdentifiers":[{}]},{"creatorNames":[{"creatorName":"板橋, 家頭夫"}],"nameIdentifiers":[{}]}]},"item_files":{"attribute_name":"ファイル情報","attribute_type":"file","attribute_value_mlt":[{"accessrole":"open_date","date":[{"dateType":"Available","dateValue":"2019-07-26"}],"displaytype":"detail","filename":"77_74.pdf","filesize":[{"value":"397.7 kB"}],"format":"application/pdf","licensetype":"license_note","mimetype":"application/pdf","url":{"label":"77_74.pdf","url":"https://showa.repo.nii.ac.jp/record/140/files/77_74.pdf"},"version_id":"3693f3d6-f3a6-475b-b3df-eab1a4d42af1"}]},"item_language":{"attribute_name":"言語","attribute_value_mlt":[{"subitem_language":"jpn"}]},"item_resource_type":{"attribute_name":"資源タイプ","attribute_value_mlt":[{"resourcetype":"departmental bulletin paper"}]},"item_title":"強制オシレーション法を用いた極低出生体重児の学童期の呼吸機能評価 ","item_titles":{"attribute_name":"タイトル","attribute_value_mlt":[{"subitem_title":"強制オシレーション法を用いた極低出生体重児の学童期の呼吸機能評価 "}]},"item_type_id":"10002","owner":"1","path":["13"],"pubdate":{"attribute_name":"公開日","attribute_value":"2017-08-25"},"publish_date":"2017-08-25","publish_status":"0","recid":"140","relation_version_is_last":true,"title":["強制オシレーション法を用いた極低出生体重児の学童期の呼吸機能評価 "],"weko_creator_id":"1","weko_shared_id":-1},"updated":"2023-05-15T13:11:18.781899+00:00"}
