{"created":"2023-05-15T11:21:27.383019+00:00","id":123,"links":{},"metadata":{"_buckets":{"deposit":"40454f4f-f16b-4ddb-b267-0f61c7d9783f"},"_deposit":{"created_by":1,"id":"123","owners":[1],"pid":{"revision_id":0,"type":"depid","value":"123"},"status":"published"},"_oai":{"id":"oai:showa.repo.nii.ac.jp:00000123","sets":["1:6:12:14"]},"author_link":["589","590","588","585","586","587"],"item_10002_biblio_info_7":{"attribute_name":"書誌情報","attribute_value_mlt":[{"bibliographicIssueDates":{"bibliographicIssueDate":"2017-04"},"bibliographicIssueNumber":"2","bibliographicPageEnd":"202","bibliographicPageStart":"194","bibliographicVolumeNumber":"77","bibliographic_titles":[{"bibliographic_title":"昭和学士会雑誌"}]}]},"item_10002_description_6":{"attribute_name":"抄録","attribute_value_mlt":[{"subitem_description":"閉塞性睡眠時無呼吸症候群（Obstructive sleep apnea syndrome：OSAS）は心血管イベントを起こす独立する危険因子と言われている．OSASに対し持続式陽圧呼吸療法（Contin?uous positive airway pressure：CPAP）は確立された治療法だが，左心機能に対する改善効果は明らかではない．これまでのCPAP治療の左心機能に対する報告はほとんどが従来の心エコー図法を用いての評価であり，われわれは微細な変化を評価可能な2D speckled tracking echocardiographic imaging 法 （2D-STE）により左心機能がどのように変化するか検討した．対象は昭和大学病院で睡眠時ポリソムノグラフィを施行し無呼吸低呼吸指数15回/時間以上の中等から重症のOSASと診断された，壁運動異常を有さない（EF≧55％）62例（女性46例，平均61±13歳）．対象をCPAP治療の使用状況により，Good-CPAP群 （26例，1か月に21日以上かつ1日4時間以上使用），Fair-CPAP群 （20例，1か月に21日未満もしくは1日4時間未満使用） ，Non-CPAP群 （16例，CPAP未使用）の3群に分けて左室収縮・拡張機能，動脈硬化所見および身体所見の治療開始前と開始後平均2.1±0.5年後の変化を比較検討した．治療開始後，治療開始前と比較し収縮期血圧はGood-CPAP群で有意に改善した（vs Non-CPAP ［p＝0.0342］，vs Fair-CPAP ［p＝0.0424］）．CAVI（Cardio ankle vascular index）値は3群間で変化は見られなかった．従来の心エコー図検査では左室拡張末期径，左室収縮末期径，左室駆出率，左房圧での変化は3群間で差は見られなかったが，左房径はGood-CPAP群がNon-CPAP群に比べ有意に縮小していた［p＝0.005］．2D-STEではLongitudinal strainではGood-CPAP群がFair-CPAP群・Non-CPAP群に比べ有意に改善が見られた［p＝0.0128，p＜0.0001］．Strain rateでの解析でも収縮能［p＝0.0274，p＝0.0002］・拡張能［p＝0.0288，p＜0.0001］ともに同様の結果であった．Radial strainでは3群間で有意差はなかったが，strain rateではGood-CPAP群がNon-CPAP群に比べ収縮能［p＝0.0087］・拡張能［p＝0.022］ともに改善していた．Circumferential strainはGood-CPAP群がNon-CPAP群に比べ有意に改善していた［p＝0.028］．Strain rateでもGood-CPAP群がNon-CPAP群に比べ収縮能［p＝0.0039］・拡張能［p＝0.0119］ともに有意に改善していた．OSAS症例ではCPAP治療を推奨時間使用することで血圧を下げることが可能である．また2D-STEにより従来の心エコー図で評価できなかった左室収縮能，拡張能が改善することが正確に評価することができた．特に長軸方向が顕著であり，CPAPを推奨時間行うことが重要だと言える．\n","subitem_description_type":"Abstract"}]},"item_10002_publisher_8":{"attribute_name":"出版者","attribute_value_mlt":[{"subitem_publisher":"昭和大学学士会"}]},"item_10002_relation_14":{"attribute_name":"DOI","attribute_value_mlt":[{"subitem_relation_type_id":{"subitem_relation_type_id_text":"10.14930/jshowaunivsoc.77.194"}}]},"item_10002_source_id_9":{"attribute_name":"ISSN","attribute_value_mlt":[{"subitem_source_identifier":"2187-719X"}]},"item_10002_version_type_20":{"attribute_name":"著者版フラグ","attribute_value_mlt":[{"subitem_version_type":"VoR"}]},"item_creator":{"attribute_name":"著者","attribute_type":"creator","attribute_value_mlt":[{"creatorNames":[{"creatorName":"福岡, 裕人"}],"nameIdentifiers":[{}]},{"creatorNames":[{"creatorName":"茅野, 博行"}],"nameIdentifiers":[{}]},{"creatorNames":[{"creatorName":"松井, 泰樹"}],"nameIdentifiers":[{}]},{"creatorNames":[{"creatorName":"太田, 礼"}],"nameIdentifiers":[{}]},{"creatorNames":[{"creatorName":"安達, 太郎"}],"nameIdentifiers":[{}]},{"creatorNames":[{"creatorName":"小林, 洋一"}],"nameIdentifiers":[{}]}]},"item_files":{"attribute_name":"ファイル情報","attribute_type":"file","attribute_value_mlt":[{"accessrole":"open_date","date":[{"dateType":"Available","dateValue":"2019-07-26"}],"displaytype":"detail","filename":"77_194.pdf","filesize":[{"value":"463.2 kB"}],"format":"application/pdf","licensetype":"license_note","mimetype":"application/pdf","url":{"label":"77_194.pdf","url":"https://showa.repo.nii.ac.jp/record/123/files/77_194.pdf"},"version_id":"b6b83d21-2d5b-490f-b7d4-1bb3affd6e14"}]},"item_language":{"attribute_name":"言語","attribute_value_mlt":[{"subitem_language":"jpn"}]},"item_resource_type":{"attribute_name":"資源タイプ","attribute_value_mlt":[{"resourcetype":"departmental bulletin paper"}]},"item_title":"長期間の持続性陽圧呼吸療法は閉塞性無呼吸症候群患者の左心機能への影響を改善し，2-dimensional speckled tracking echocardiographic imaging法は評価方法として有効である ","item_titles":{"attribute_name":"タイトル","attribute_value_mlt":[{"subitem_title":"長期間の持続性陽圧呼吸療法は閉塞性無呼吸症候群患者の左心機能への影響を改善し，2-dimensional speckled tracking echocardiographic imaging法は評価方法として有効である "}]},"item_type_id":"10002","owner":"1","path":["14"],"pubdate":{"attribute_name":"公開日","attribute_value":"2017-10-05"},"publish_date":"2017-10-05","publish_status":"0","recid":"123","relation_version_is_last":true,"title":["長期間の持続性陽圧呼吸療法は閉塞性無呼吸症候群患者の左心機能への影響を改善し，2-dimensional speckled tracking echocardiographic imaging法は評価方法として有効である "],"weko_creator_id":"1","weko_shared_id":-1},"updated":"2023-05-15T13:11:48.176083+00:00"}
